Travel Clinic

Please try to give us 6-8 weeks notice if you are travelling abroad and think that you may need vaccinations.  The 'Fit For Travel' website gives up to date information which you will find useful.

Download our Travel Health Questionnaire (travel clinic form).

Please give as much detail as you can about your planned travel.  When you return this form to us, the Practice Nurse will determine whether any vaccinations are required and will contact you - by phone and/or email - to arrange an appointment.  If vaccinations are needed, we will order these in and have them delivered to the surgery.
